Under the general supervision of their collaborative physician; plans, directs, and evaluates patient health care in primary health care services. Examines and treats chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses. Performs selected laboratory testing, referrals to physicians as needed, writes prescriptions, and performs office procedures within the scope of expertise, protocols, and available equipment. Actively participates in physician lead provider care teams. In NP staffed sites may lead a provider care team.
ESSENTIAL RESPONSIBILITIES:
Provides advanced nursing care in collaboration with physician and/or other members of health care team.
Provides care based on established guidelines
Performs physical examinations and record patient's health and psychosocial and family histories; orders, interprets, and evaluates diagnostic studies and tests to assess patient's clinical problems and health care needs; initiates, evaluates, and modifies patient's therapeutic plan; evaluates, interprets, and records the developmental status of patient and family in order to plan and implement interventions; engages in decision making and implementation of therapeutic plans in collaboration with other members of the health care team.
Supervises, directs, and evaluates nursing staff; directs the ordering of pharmaceuticals, medical supplies and equipment; maintains accurate records, reports and statistics related to the health care of patient and the Center's funding regulation requirements.
Work with the Chief Executive Officer in regard to non-medical duties and is subject to Center's policies and procedures.
Actively participates in all PCMH activities including patient care huddles and pre-visit planning, compilation of patient care plans, initiation of patient education and patient self management goals
Actively participates in the quality improvement activities of the care team
Must have a current certification in basic cardiac life support (BCLS) or advance cardiac life support (ACLS).
Participate in the education of students rotating with-in FPHC.
Performs other related duties as requested by the immediate supervisor and/or CMO.

PERFORMANCE STANDARDS:
Performance will be evaluated by the Lead Nurse Practitioner or Collaborative Physician annually wherein an overall satisfactory rating is considered as a minimum acceptable level of performance.
HEALTH AND SAFETY:
Must adhere to safety policies and procedures.
Must receive Safety and Hazardous Communication training annually.
Must exercise universal precautions and comply with safety standards and procedures.
Knowledgeable of OSHA and CLIA requirements to assure compliance.

Adheres to Health Insurance Portability and Accountability Act (HIPAA Privacy Rule) policies and procedures.
Must successfully complete Privacy Rule compliance training annually or as revisions are made to the policies and procedures.
Must comply with Privacy Rule guidelines by learning to protect FPHC patient's medical privacy.
Must comply with Privacy Rule guidelines by appropriately maintaining our patient information in compliance with national standards.
Must comply with Privacy Rule guidelines by providing appropriate security of FPHC patient records

EDUCATION AND EXPERIENCE
: Master degree in nursing science from an accredited college or university. Licensed and certified to practice in the State of Alabama. Clinical practice experience preferred.
CERTIFICATIONS, LICENSURES AND REGISTRATIONS:
Alabama Board Nurse Licensure, BCLS or ACLS Certification, Board Certified
OTHER QUALIFICATIONS
: Proficiency in English. Cultural competency. Possess outgoing and friendly personality with ability to establish effective working relationships with other employees. Must have ability to establish good rapport and gain confidence of patients.
PHYSICAL/MENTAL DEMANDS:
Requires working under stressful conditions and irregular hours.
Must be able to express ideas clearly and concisely; and exercise balanced judgment in evaluating situations and making decisions.
Work requires sitting, standing, stooping, bending, driving and a considerable amount of walking, moving and examining objects at high and low reach, and reading.
