Nurse Practitioner (FNP or ANP) - Manhattan or Queens

Join a highly respected community health center known for exceptional, mission-driven care. We're seeking an experienced NP to provide comprehensive outpatient primary care and support population health efforts across diverse communities.

About the Role

You'll manage a panel of adult patients, providing full-scope primary care including acute and chronic disease management, preventive care, mental health screening and treatment, and health promotion. Key Responsibilities

Evaluate and manage acute and chronic conditions Perform initial assessment and treatment of depression, anxiety, and other common mental health needs Order and interpret diagnostic tests using evidence-based principles Prescribe medications and treatments within licensure and best-practice guidelines Counsel patients on treatment plans, lifestyle changes, and preventive screenings Coordinate specialist referrals as needed Maintain accurate, timely EMR documentation (eClinicalWorks) Participate in call rotation, team meetings, pre-visit planning, QI initiatives, and peer review Uphold regulatory standards, maintain required certifications, and ensure safe, high-quality care Work effectively with diverse populations, including recent immigrants

Qualifications

MSN or DNP; completion of accredited NP program Certified FNP or ANP Active NYS NP license, DEA, BLS, and infection control certificate Minimum 2 years as a Nurse Practitioner in an ambulatory or FQHC setting required Strong preference for experience in women's health, PrEP/PEP, STI testing/treatment, birth control management, and basic mental health prescribing Telehealth-only or nursing home/rehab backgrounds cannot be considered Excellent communication and patient engagement skills

Salary

$135,000-$160,000 DOE + outstanding benefits
