Job Overview

We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. Wound care experience a plus! The ideal candidate will conduct comprehensive wound assessments, including pressure injuries, diabetic foot ulcers, venous and arterial ulcers, surgical wounds, and traumatic wounds.

Duties

As a Nurse Practitioner, you will be responsible for:

Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams. Perform advanced wound care procedures such as debridement, dressing changes, negative pressure wound therapy, and infection management. Educate patients, caregivers and facility staff on wound care best practices and prevention strategies. Prescribe medications, treatments, and durable medical equipment as appropriate. Monitor wound healing progress and adjust treatment plans as needed. Maintain accurate, timely, and compliant electronic medical records. Coordinate with referring physicians, home health agencies, and other healthcare providers to ensure continuity of care. Travel within the designated service area, managing your schedule to optimize patient care and efficiency.

Experience

Candidates should possess the following qualifications:

Active Nurse Practitioner license (state-specific, in good standing). Board certification as an NP (Family, Adult-Gerontology, or Acute Care preferred). Minimum 1-2 years of experience in wound care, dermatology, surgery, or related specialty (WOCN certification a plus). Current DEA registration (if prescriptive authority required). Valid driver's license and reliable transportation. Strong clinical assessment, critical thinking, and decision-making skills. Excellent communication and patient education abilities. Comfortable working independently in divers community settings.
