RESPONSIBILITIES

Certified Registered Nurse Practitioner (CRNP) is responsible for resident health and care within our skilled nursing/personal care facility Maintains compliance with PPHP Model of Care, Completion of Comprehensive Health Record (CHR), and reviewing appropriate HEDIS measures Assigning patient risk per PPHP policy Document any Transfer of Care (TOC) Review all testing, labs, and consults Complete walking rounds and communicate regarding resident concerns Report and log hospital admissions and any unplanned transfers Coordinate and complete employee physicals

REQUIREMENTS

Completion of an accredited Nurse Practitioner program Current and unrestricted NP license in PA
