At HomeMD, our mobile Nurse Practitioners deliver comprehensive healthcare directly to seniors in their residence--whether private homes, assisted living, or similar settings. By bringing care to the patient, we reduce travel barriers and improve continuity of medical support for older adults.
Primary Responsibilities
Patient Health Monitoring & Routine Visits
Conduct routine physical assessments and wellness exams
Monitor blood pressure, glucose control, cognitive status, cholesterol, and other clinical measures
Adjust treatment plans when changes in condition are identified
Chronic Care Support
Oversee long-term health issues such as diabetes, hypertension, arthritis, heart disease, dementia, and more
Tailor treatment recommendations to each patient's ongoing needs
Medication-Related Care
Review existing medication regimens for risks, interactions, and effectiveness
Coordinate medication adjustments with pharmacies and collaborating providers
Provide education to residents and caregivers to ensure safe usage
Comfort and Supportive Care
Participate in care planning for patients facing terminal or progressive health decline
Focus on comfort, symptom management, quality of life, and support for families
Preventive Care Initiatives
Administer recommended vaccines and order preventive screenings
Promote wellness practices related to falls, nutrition, mobility, and infection prevention
Acute or On-Demand Tele Support
Conduct same-day evaluations when sudden illness or injury occurs
For part-time providers, a support team assists with urgent patient needs
Collaborative Care Planning
Build and maintain communication with physicians, specialists, families, and community care staff
Ensure clear documentation and follow-up

Qualifications
Active Michigan NP license
Graduate-level nursing degree (MSN, DNP) and board certification
Experience with geriatrics, complex chronic conditions, urgent care, or wound/palliative care is a strong advantage
Strong assessment and communication skills
Ability to practice independently with remote clinical support
