Nurse Practitioner (General Practice)

Four Squared Solutions is seeking a dedicated and compassionate Nurse Practitioner (NP) to join our clients General Practice / Primary Care team in Lake Mary, Florida . The ideal candidate will provide comprehensive, patient-centered care to individuals and families across all age groups, focusing on prevention, wellness, and the management of chronic and acute conditions.


Employment Type: Full-time


Schedule: MondayFriday 9:00 a.m.5:00 p.m.


As an integral part of our care team, the NP will deliver high-quality clinical services while fostering long-term patient relationships and promoting community health.

Key Responsibilities

Conduct comprehensive health assessments, physical exams, and preventive screenings. Diagnose and manage acute and chronic illnesses such as hypertension, general, diabetes, COPD, asthma, and thyroid disorders. Order and interpret laboratory tests, diagnostic studies, and imaging. Develop individualized treatment and management plans. Prescribe medications and therapeutic interventions within state scope of practice. Provide patient education on disease prevention, lifestyle modification, and medication adherence. Perform routine in-office procedures (e.g., joint injections, immunizations, pap smears, wound care). Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care. Maintain accurate and timely documentation in the EMR. Participate in quality improvement initiatives and health promotion programs.

Qualifications

Masters or Doctoral degree from an accredited Nurse Practitioner program. Current and unrestricted Nurse Practitioner license in [Insert State]. National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care NP (AGNP). Minimum of 3years of primary care experience preferred. Excellent clinical, communication, and patient counseling skills. Proficient in EMR systems and telehealth platforms.
