We are seeking a highly skilled and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive clinical expertise across various specialties, including acute and chronic care, emergency medicine, geriatrics, pediatrics, and behavioral health. This role involves providing comprehensive patient assessments, diagnosing conditions, developing treatment plans, and collaborating with multidisciplinary teams to deliver exceptional patient-centered care. Experience with EMR/EHR systems is essential. The Nurse Practitioner will also be involved in medical documentation, medication administration, and patient education within outpatient and inpatient settings. Candidates with specialized skills in areas like telehealth, GLP1 injections and suboxone treatment are encouraged to apply.
Responsibilities
Conduct thorough physical examinations and patient assessments across diverse populations including pediatrics, geriatrics, and acute care patients.
Develop and implement individualized treatment plans based on clinical evaluations.
Administer injections.
Manage acute pain cases utilizing advanced pain management techniques and IV infusion therapies.
Perform diagnostic evaluations using laboratory testing.
Utilize EMR/EHR systemfor accurate medical documentation and coding (ICD-9/10, CPT).
Provide telehealth consultations to expand access to care while maintaining HIPAA compliance.
Participate in discharge planning, case management, utilization review, and post-acute care coordination.
Manage complex cases
Educate patients on health coaching topics such as nutrition, family planning, medication adherence, and disease prevention.
Ensure infection control practices are followed diligently to maintain sterile processing standards.
Skills
Proven experience with EMR/EHR system; proficiency in medical documentation and ICD coding is required.
Strong clinical skills across multiple specialties including internal medicine, riatrics (memory care & Alzheimer's care), behavioral health, occupational health, and post-acute settings.
Knowledge of pharmacology related to psychopharmacology and infection control protocols.
Excellent communication skills for patient education and interdisciplinary collaboration.
Familiarity with medical imaging modalities and diagnostic evaluation techniques.
Ability to work independently within a multidisciplinary team while adhering to NCQA standards and hospital policies.

Pay: $50,000.00 - $122,228.17 per year
