Overview


We are seeking a highly skilled and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive clinical expertise across various specialties, including, primary care, geriatrics, pain management within the Hospice environment. This role offers an opportunity to provide comprehensive patient care in diverse settings, utilizing advanced skills in medical assessment, diagnosis, treatment planning, and patient education. The Nurse Practitioner will work closely with physicians, nurses, and other healthcare professionals to deliver high-quality care while adhering to all regulatory standards and maintaining patient safety. Proficiency with EMR/EHR systems such as Matrix is preferred and is the current system for documentation and patient management.

Responsibilities



Conduct thorough physical examinations and patient assessments across outpatient settings . Develop and implement individualized treatment plans based on diagnostic evaluations and clinical findings. Evaluate pain management needs and refer to physician for follow up. Infectious diseases, hospice & palliative medicine needs, and complex chronic conditions like diabetes or cardiovascular issues are also part of any evaluation. Assist with diagnostic evaluations using laboratory tests, medical imaging, sonography, and specimen collection/processing. Document all patient interactions accurately within EMR/EHR systems. Educate patients on health coaching topics including nutrition, medication adherence, disease management, family planning, and post-discharge care. Collaborate with multidisciplinary teams for discharge planning, utilization review, case management, and working with special populations. Maintain current knowledge of medical terminology, CPT/ICD coding (ICD-9/10),

Requirements



Valid Nurse Practitioner license with current certification to practice independently in the state of employment. Ability to manage complex cases involving pain management or infectious disease care; familiarity with hospice & palliative medicine is a plus. Skilled in diagnostic evaluation including laboratory testing (phlebotomy), medical imaging interpretation (ultrasound), and specimen processing. Knowledge of CPT coding & ICD coding (ICD-9/10) for accurate documentation and billing purposes. Excellent communication skills for effective patient education and interdisciplinary collaboration; commitment to maintaining HIPAA compliance at all times.
