SPECIFIC DUTIES:




The Nurse Practitioner (NP) provides high-quality, patient-centered primary care services, including assessment, diagnosis, treatment, and preventive care across the lifespan. The NP functions collaboratively within a multidisciplinary team to ensure comprehensive patient management consistent with medical standards, state and federal regulations, and CARF/DATEP/Medicaid/Medicare compliance.


Additional duties include performing mental health screenings, assessing psychiatric conditions, and collaborating with licensed mental health professionals for treatment planning and crisis intervention. The NP also provides integrated behavioral and mental health support, performing evaluations, medication management, and coordination with behavioral health teams to ensure holistic care for individuals experiencing co-occurring medical and mental health conditions.


Essential Functions Include:




Conduct comprehensive physical examinations and obtain detailed health histories.


Diagnose and manage acute and chronic health conditions in accordance with evidence-based guidelines.


Develop and implement individualized treatment plans, including prescribing medications and ordering diagnostic tests.


Provide preventive health education and counseling to patients and families.


Collaborate with physicians, RNs, behavioral health specialists, and other healthcare team members to coordinate integrated care.


Monitor patient progress and modify treatment plans as appropriate.


Document all patient encounters accurately and timely within the electronic health record (EHR) system.


Participate in quality assurance, peer review, and performance improvement initiatives.


Ensure compliance with CARF standards, HIPAA, OSHA, and Georgia Medicaid/Medicare regulations.


Maintain current knowledge of clinical best practices, pharmacology, and public health trends.


Provide consultation and training to nursing and support staff as needed.


Participate in case conferences, staff meetings, and interdisciplinary rounds.


MINIMUM EDUCATION / EXPERIENCE:




Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited institution.


Current and unrestricted Nurse Practitioner license in the State of Georgia.


National certification as a Family Nurse Practitioner (FNP-C or FNP-BC) preferred.


Minimum of 2 years of clinical experience in primary care, family practice, or behavioral/mental health settings preferred.


Experience working with diverse populations, including individuals with chronic conditions, mental health, or substance use disorders.


Skills and Competencies:


Strong diagnostic, clinical, and decision-making skills.


Excellent communication, leadership, and interpersonal abilities.


Competency with EHR systems and telehealth platforms.


Knowledge of Medicaid/Medicare billing and coding preferred.


Ability to work both independently and collaboratively within a team-based model.


Commitment to ethical practice, cultural competence, and continuous improvement.


Licensure and Certifications:




Current Georgia RN and APRN licensure.


DEA registration (or eligibility).


BLS and ACLS certification (required).


ANCC/AANP certification preferred.


SCHEDULE OF PROGRAM ACTIVITY:




This position operates within a standard clinical setting providing direct patient care. The schedule may vary based on patient needs and facility requirements. May require occasional travel between facility sites or community events. Regular standing, walking, lifting up to 25 lbs, and use of medical instruments.
