We conduct Comprehensive Health Screenings, identifying Heart and Lung Disease, and early identification of Cancer. We are seeking a skilled and motivated RN Case Manager to join our healthcare team. As an RN Case Manager, you will play a crucial role in providing oversight to each department we serve in multiple states, ensuring no firefighter is left behind. Responsibilities

Coordinating healthcare services, appointments, and managing a roster of patients that must be seen annually. Ensuring accurate and up-to-date documentation of patient information and care plans. Monitoring and evaluating patient progress and outcomes when a patient is deemed not fit for duty. Ensure timely and effective communication with patients, healthcare providers, and payers ( Fire or Law enforcement agencies) Report out weekly updates to the leadership team. Client communication and coordination of annual physical projects Bachelor's or Master's degree in Nursing. Certification in Case Management (CCM or ACM) preferred Experience in a similar role in healthcare settings. Familiarity with electronic health records (EHR) systems. Strong knowledge of healthcare regulations and compliance. Excellent organizational and time-management skills. Advanced clinical assessment and critical-thinking abilities. Ability to work independently and as part of a team. Strong communication and interpersonal skills. Strong working knowledge of Microsoft products, including Excel. You will use Excel and online software programs extensively.
