• Conduct comprehensive assessments of referred member's needs and eligibility.

• Apply applicable criteria and guidelines, ensuring appropriate administration of benefits.

Critical Information

• Work hours are 8am-5pm Monday through Friday.
• This is an in-person, patient-facing role.
• Requires up to 75% travel for member-facing visits and documentation at home.

Education/Licenses Needed

• RN with current unrestricted state licensure.
• Case Management Certification (CCM) preferred.
• High School Diploma or GED required.
