Duties
  • Responsible for planning, development and implementation of a clinically based program designed to provide high quality care coordination to effect seamless health care delivery.
  • Identify problems with health care access and utilization in both military and civilian sectors and recommends alternative to overcome these difficulties.
  • Review network referrals to ensure the Center for Medicare and Medicaid Services (CMS), Veterans Affairs and/or TRICARE requirements are met prior to patient transfer and disengagement.
  • Interface with local, state and Federal agencies responsible for financing alternatives which DOD beneficiaries may be entitled to receive.
  • Implement initiatives and/or changes to the TRICARE benefit to meet patient needs in a quality conscious, resource efficient manner.
  • Coordinate with Nursing staff, Social Work Service discharge planners, Veteran Affairs care coordinators, sister services and Patient Administration to identify patients requiring health care services outside the Military Treatment Facility.
  • Assist in identifying other options for care when patients no longer require acute hospitalization, discharge planning and home health services, hospice, rehabilitation services, long term care placement, and obtaining durable medical equipment

  • This position requires Basic Life Support training certification from, or in accordance with, the American Heart Association and American Red Cross. Current Advanced Life Support or other advanced certification does not supersede BLS completion.
  • The incumbent must make every attempt to report for work on time and/or remain on duty during severe weather conditions.
  • The duties of this position meet the criteria for compliance with Federal and Department of the Army Drug Free Workplace Programs. This position is subject to a pre-employment screening, and random testing thereafter.

Basic Requirement for Nurse (Clinical/Medical-Surgical): Degree: A graduate or higher-level degree, bachelor\'s degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.).

Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.

Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status. Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

Experience: A diploma, associate degree, or bachelor\'s degree in nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-09 grade level or above) which includes managing referrals, care coordination, collecting clinical data, managing reports, and providing patient or staff education.

OR

Education: Successful completion of two and a half years of progressively higher-level graduate education leading towards a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
