We are looking for a total of seven individuals to work within various assigned regions within the State of Utah in order to provide non-medical case management to make sure Ryan White Part B clients enter and remain in HIV medical care and have access to and are adherent on their HIV medications. This will be groundbreaking work that you will be participating in as the State of Utah is looking to now offer services that were previously unavailable to many Utahans living in rural areas.

In this role you will support the AIDS Drug Assistance Program (ADAP) and ensure that clients enrolled in the Ryan White Part B Program are able to access core and support services offered by the Program in an efficient and timely manner. Your role will be a new one for the State of Utah and you will be working with clients (especially in rural areas) that have not had direct access in the past to HIV treatments and counseling.   • There are seven open positions which will be assigned to various regions as follows (assignments will be made as interviews are completed):
  • Northern Utah (x1)
  • Wasatch Front (x4)
  • Central Utah (Richfield region) (x1)
  • Southern Utah (Cedar City / St. George region) (x1)
  • This is a continuous recruitment and has an open closing date. However, this recruitment is subject to close at any time at the discretion of the hiring manager.
Principal Duties
As a Non-Medical Case Manager you will;
  • Provide case management in accordance with the Case Management Service Standards for the Utah Ryan White Part B Program.
  • Perform client intake
  • Assist in application and re-certification for the Part B Program
  • Assess needs utilizing assessments as provided by the Program
  • Develop care plan
  • Coordinate care and benefits
  • Provide and track referrals
  • Provide internal and external resources
  • Perform follow-up as needed and indicated by acuity and assessment scores.
  • Perform any other duties where required.
Our Ideal Candidate has:
  • Working knowledge of HIV
  • Knowledge of or experience with Ryan White Part B
  • Familiarity with social service and support systems within Utah
  • Experience with ClientTrack data management solution.
  • The ability to work independently with minimal supervision.
  • A working knowledge of web-based applications and Google Workspace, Microsoft Office, or similar.
Preference will be given to applicants with:
  • One year of paid employment/professional experience providing case management services, excluding internships, AND AT LEAST one of the following: additional items:
  • A Bachelor\'s Degree of Social Work (or closely related degree)
  • A Bachelor\'s Degree in Health or Human Services (or closely related degree).
  • Five years of paid employment/professional experience providing case management services, excluding internships.

Working Conditions:
  • The typical office setting for this job is adequately lighted, heated and ventilated. However, you may encounter some risks that are associated with any office setting, for example, the safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
  • Standard Schedule: Monday- Friday, 8:00 am - 5:00 pm Additional hours may be needed to factor in for traveling times.
Physical requirements:
  • Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
