Job Description

: Integrated Care Coordinator RNwill engage with behavioral healthcare clients to ensure clients are receiving the most effective and appropriate care in managing their behavioral health and primary care needs. Responsible for MOUD nursing duties and management of all drug samples and drug closets throughout the region including inventory control, maintaining medication sample distribution logs, and purging all expired samples and medical supplies

Role and Responsibilities



Care Coordination and Clinic Duties:



Rooming clients, vitals, ht/wt Medication administration & tracking Provide medication education to clients Maintain medical equipment in proper working order; maintain confidentiality and proper boundaries Obtain urine drug screens, HCG screens or other screens as indicated; Lab to pick up when indicated Venipuncture for baseline labs and labs as needed; Lab to pick up Engage child, adolescent, and adult clients with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), and Substance Use Disorder (SUD) diagnoses in a structured, collaborative relationship that empowers each client to gain the necessary skills in a variety of areas related to recovery and self-management of their mental health, substance abuse, and developmental disabilities. Promote environmental and psycho-social health and maximize independence to improve and maintain lifelong well-being and functional status. Provide evidence-based care concerning complex cases of vulnerable individuals with SMI, SED, SUD, and/or developmental/intellectual disabilities. Identify and address risks and current or potential gaps in care to prevent unnecessary deterioration that could result in hospitalizations, emergency room visits, and/or loss of independence in vocational, academic, and/or social functioning. Provide education to clients to promote improved functioning skills and development. Complete requests for prior authorizations as indicated to provide necessary services. Participate in staff meetings and consultation with other clinicians to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services. Act as an advocate to clients to ensure they receive necessary services in the community. Develop knowledge and establish good working relationships with community support resources and services within the community. Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations. Complete timely and accurate documentation within an electronic health record and other documentation as required of the position. Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills. Maintain flexible hours to meet agency and client needs. Complete initial Nursing Assessment with new admissions, as applicable Order and maintain clinic supplies Knowledge and delivery of detox protocols. Assessment/Screenings (COWS, CIWA-Ar, ACSA, etc.) with appropriate clients per protocol. Run KASPER reports. Coordinate and support Medical providers with assessments and medication management. Complete prior authorization forms for Insurance, Medicaid, and Medicare Part D recipients. Provide nursing assistance for all patients and MD's including administering injections, prescription distribution to clients (under MD direction). Completing all treatment plan updates and psychosocial addendums for clients under MD only care. Other duties as assigned.

Qualifications and Education Requirements

Registered Nurse with unrestricted license in Kentucky and a minimum of 3 years SUD, Psychiatric our MOUD experience preferred.
