SUBMISSION REQUIREMENTS:
- Psych/Behavioral Health Experience - Required
- Experience working with child and adolescent populations - Required
- CPI - please confirm they are willing to obtain onsite if they don\'t already have this certification. - Required
- COVID vaccination and Booster - Required
- Please outline what patient populations your candidate has experience with.
CERTIFICATION REQUIREMENTS:
- BLS
- CPI
STATE LICENSE REQUIREMENTS:
- Vermont
ADDITIONAL LICENSE REQUIREMENTS:
- VT license must be active at time of submission!
