Registered Nurse
for our Hurricane office location. This office is located in Hurricane WV and provides service to the counties of Cabell, Wayne, Putnam and Kanawha.

PRN Mental Health Registered Nurse
with Personal Touch, you will provide patient care, and ensure that all patients receive optimum quality care, and be an integral part of the interdisciplinary team.
Position Summary
The Mental Health Registered Nurse (RN) provides direct skilled nursing care and mental health interventions to patients in the home setting under the direction of the physician and in accordance with the Certified Home Health Agency's (CHHA) policies, procedures, and the Medicare Conditions of Participation (CoPs). The RN specializes in psychiatric and behavioral health conditions, assessing, planning, implementing, and evaluating care to promote stabilization, recovery, and optimal functioning while ensuring compliance with Medicare, Medicaid, and state regulations.
ESSENTIAL JOB FUNCTIONS/ RESPONSIBILITIES
1. Assessment & Planning
Perform the initial comprehensive assessment, including the Outcome and Assessment Information Set (OASIS) when required by Medicare, within required timeframes.
Conduct physical, psychiatric, psychosocial, and environmental assessments.
Identify mental health diagnoses, symptoms, and risk factors for self-harm or harm to others.
Collaborate with the physician and interdisciplinary team to develop an individualized Plan of Care (POC) that addresses both mental health and medical needs.
Review and revise the POC at least every 60 days or as the patient's condition changes.
2. Skilled Nursing & Mental Health Care
Provide direct skilled nursing services including medication administration (including psychotropic medications), symptom monitoring, wound care, and health teaching.
Deliver psychiatric nursing interventions such as crisis stabilization, coping skill development, and relapse prevention.
Monitor and manage medication side effects and coordinate adjustments with the prescribing provider.
Educate patients, caregivers, and families about the disease process, medication adherence, symptom management, and available resources.
Provide suicide risk screening, mental status examinations, and behavioral assessments as indicated.
3. Documentation & Compliance
Complete and submit all documentation--including OASIS--on the day of the visit or within the agency's required timeframe to meet Medicare and state requirements.
Document all skilled interventions, patient/caregiver education, and measurable progress toward POC goals.
Report changes in the patient's condition to the physician and Director of Nursing immediately per CoP 484.60(c).
Maintain compliance with HIPAA, Medicare CoPs, and agency policies.
4. Coordination & Communication
Participate in interdisciplinary case conferences to review patient progress, modify care plans, and ensure continuity of care.
Collaborate with social workers, therapists, home health aides, and other team members to address social, emotional, and environmental needs.
Facilitate transitions of care to and from inpatient psychiatric or medical facilities.
5. Quality & Safety
Adhere to infection prevention and control measures per CoP 484.70.
Participate in the CHHA's
Quality Assessment and Performance Improvement (QAPI)
activities.
Report patient incidents, unusual occurrences, and safety concerns promptly according to agency policy.
Qualifications
Required:
Current and unencumbered RN license in the state of practice.
At least one (1) year of recent nursing experience, including at least six (6) months in psychiatric or mental health nursing.
Completion of a psychiatric nursing orientation program approved by the agency.
Current CPR/BLS certification.
Valid driver's license, automobile insurance, and reliable transportation.
Preferred:
Bachelor's degree in Nursing (BSN).
Certification in Psychiatric-Mental Health Nursing (RN-BC).
Prior home health nursing experience in a Medicare-certified CHHA.
* Working knowledge of OASIS data collection and Medicare documentation requirements.
