The Memory Care Coordinator is responsible for the overall leadership, operation, and quality of care within the Memory Care neighborhood. This role ensures a safe, supportive, and engaging environment for residents while providing direct supervision, training, and support to medication technicians and caregivers. The Memory Care Coordinator works hands-on as needed and ensures compliance with all state Personal Care Home (PCH) regulations and community standards.
Key Responsibilities:
Provide day-to-day leadership of the Memory Care neighborhood, including supervision, training, coaching, and discipline of medication technicians and caregivers
Monitor residents continuously for changes in health status, behavior, and overall well-being, communicating concerns appropriately
Work on the floor as needed to support staffing, resident care, and unit operations
Maintain knowledge of state PCH regulations and ensure all guidelines are consistently followed within the unit
Complete and oversee required resident evaluations and assessments, including RASP/DME, Level of Care (LOC), and other applicable assessments
Ensure shower schedules, laundry services, and personal care routines are followed consistently
Develop, maintain, and implement a monthly activities calendar specific to the Memory Care neighborhood
Lead and participate in unit-based activities, including structured programming, arts and crafts, and creative engagement opportunities
Promote resident-centered care and meaningful engagement tailored to cognitive abilities
Ensure a safe, secure, and supportive environment for both residents and staff
Collaborate with nursing, administration, and other departments to ensure high-quality care and service delivery
Qualifications:
Must be a licensed LPN in the state of Pennsylvania
Experience working in memory care, personal care, or a related senior living or healthcare environment
Knowledge of state Personal Care Home (PCH) regulations and memory care best practices
Strong leadership, communication, and organizational skills
Ability to work hands-on with residents and staff in a fast-paced care environment
Experience completing or overseeing resident assessments preferred
Commitment to resident safety, dignity, and quality of life
Physical & Work Requirements:
Ability to work on the floor, assist residents as needed, and respond to emergencies
Available for on-call every other weekend
