RESPONSIBILITIES



Assisting clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as outlined in the individualized Plan of Care, including bathing, dressing, grooming, toileting, and mobility. Building trusting, respectful relationships with clients and their families, maintaining confidentiality, dignity, and compassion at all times. Maintaining a safe, clean, and organized home environment to promote client health and prevent injury. Providing ongoing emotional and household support to clients and their families, such as assistance with light housekeeping, meal preparation, grocery organization, and routine paperwork. Assisting with or encouraging therapeutic exercises as directed by a licensed clinician or as detailed in the care plan. Offering consistent medication reminders and monitoring healthcare needs, while reporting concerns promptly. Promoting client independence by encouraging participation in daily routines and supporting physical and mental engagement. Stimulating mental alertness by facilitating light recreational activities, hobbies, or conversations to support overall well-being. Accurately and promptly documenting daily activities, changes in condition, mood, and any challenges or incidents during the shift. Immediately reporting changes in a client's physical, emotional, or environmental status to the supervising nurse or care coordinator Attending required in-service training, continuing education, or staff meetings in compliance with state and federal home care regulations.

Job Type: Part-time

Pay: From $17.00 per hour

Expected hours: No more than 25 per week
