Your work will support individuals across diverse settings--including inpatient hospitals, outpatient clinics, hospice care, and community-based programs--helping them navigate complex emotional and social challenges. Responsibilities



Conduct thorough patient assessments to evaluate social, emotional, and environmental factors affecting health and recovery. Develop tailored care plans that address individual needs, including mental health counseling, behavioral therapy, and crisis intervention. Provide individual and group therapy sessions utilizing techniques such as motivational interviewing, cognitive behavioral therapy, and applied behavior analysis. Collaborate with multidisciplinary teams to coordinate services for patients with various conditions including dementia, autism spectrum disorder, PTSD, addiction issues, and developmental disabilities. Manage case documentation in compliance with HIPAA regulations, ensuring accurate medical records and timely updates in electronic medical record (EMR) systems. Assist with intake procedures for new patients and facilitate connections to community resources such as assisted living or home care services. Support families through counseling on complex diagnoses like Alzheimer's disease or dementia, offering guidance on caregiving strategies and emotional support.

Qualifications



Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law. Proven experience in hospital settings, hospice care, outpatient clinics, or community health environments. Strong knowledge of medical terminology, ICD coding systems (ICD-9/10), CPT coding for billing purposes, and familiarity with EMR systems. Expertise in behavioral health disciplines including mental health counseling, crisis management, trauma-informed care, and working with individuals with disabilities or developmental delays. Experience providing specialized care such as Alzheimer's/dementia support, autism spectrum interventions, PTSD treatment, or senior/geriatric care is highly desirable. Excellent communication skills for conducting diagnostic evaluations, intake interviews, and facilitating group therapy sessions. Ability to handle sensitive situations with professionalism while maintaining strict confidentiality in accordance with HIPAA standards.
