Job Title:

Medical Social Worker for Home Health

Job Summary:

Make assessments/plans that relate directly to plan of treatment authorized by patient's physician that will help restore the significant social and emotional factors that affect the patient's health.

Education & Experience:

1. Must be a graduate of accredited school of Social Work with a master's degree.

2. Must have one year of social work experience in a healthcare setting.

3. Current member of the Academy of Certified Social Workers.

4. Must be licensed by the State of Nevada.

5. Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.

Job Function:

1. Accept referrals from the Home Health staff.

2. Will call patient/caregivers to set up a designated date and time for initial evaluation within 72 hours of the start of care date. A coordination note must be completed with explanation for the delay.

3. Review referral with Case Manager to determine the need for visit by the Medical Social Worker.

4. Perform initial evaluation and effectively plan/execute appropriate resolution/treatment.

5. Assist Case Manager, physicians, and other team members in understanding the significant social and emotional factors that relate to patient's health problems.

6. Prepare clinical progress notes of visits, document phone contacts with patient/family and Case Manager.

7. Coordinate and plan patient/family care with other team members.

8. Utilize appropriate community resources as needed.

9. Participate in discharge planning.

10. Participate in in-service training/educational programs as assigned and quality assurance conferences.

11. Follow-up for initiated programs/services.

12. Submit notes in a timely manner.

13. Must recognize complete confidentiality of all patient information.

14. Comply with infection prevention and control policies and procedures.

15. Participate in quality assurance and performance improvement activities as needed.

16. Honor patient rights.

Special Demands:

1. Must work in homes without direct supervision with the realization that errors or accidents may have serious consequences for patients.

2. Must use good judgment in recognizing signs and symptoms of unusual reactions to treatment that are relevant to the patient's health.

Job Type: Part-time

Pay: From $100.00 per hour
