- Conduct assessments and evaluations of patients\' social and emotional needs
- Develop and implement individualized care plans for patients
- Provide counseling and support to patients and their families
- Collaborate with interdisciplinary healthcare teams to coordinate patient care
- Assist patients in accessing community resources and support services
- Advocate for patients\' rights and ensure their needs are met
- Maintain accurate and up-to-date medical documentation ```Qualifications:```
- Bachelor\'s or Master\'s degree in Social Work or related field
- Valid state licensure as a Social Worker
- Experience working with individuals with autism or other developmental disabilities
- Knowledge of medical terminology and documentation practices
- Familiarity with group therapy techniques and mental health counseling strategies
- Understanding of applied behavior analysis principles
- Ability to provide crisis intervention and support during difficult situations
- Experience in senior care or child and family counseling is a plus
