Description:
The Medical Case Manager will work closely with patients living with HIV/AIDS who have multiple psychosocial and/or health related needs. Medical Case Management services include written plans of care, treatment adherence counseling, coordination, and follow-up of medical treatments, patient advocacy, and assistance in obtaining housing, financial support, legal services, transportation, social support, and any other needed services. The goals of Medical Case Management are to ensure patients with HIV/AIDS have timely access to comprehensive medical care and social services; prevent disease transmission and delay of HIV progression and to promote and support client independency.
Roles/Responsibilities:
Work in conjunction with medical care providers/pharmacists/nurses/non-medical case managers/linkage care coordinators/benefit coordinator/community health worker to ensure all patients are case managed at least twice a year.
Implement the medical treatment plan by providing counseling on medication, appointments, and other treatment adherence issues.
Verify enrollment in medical care, insurance plan and a 340B program.
Perform patient intakes by completing a FACT for every patient newly diagnosed, old diagnosed new to our organization and out of care and assist/refer to needed areas to improve patient outcomes.
Complete a patient assessment and reassessment every 6 months until no longer a managed patient.
Develop and complete at least once a year and as needed a written plan of care on every patient to include short- and long-term goals focused on attaining, maintaining and achieving positive health outcomes to include retention in care and viral suppression.
Refer/link patients to the appropriate services to within the system of care as needed and per patient request.
Complete a treatment adherence tool/follow-up at least yearly and/or as needed.
To assist patients in understanding therapeutic procedures and recommended treatment plans and in participating effectively in treatment decisions and plans.
To provide supportive and educational information about HIV and HIV medications for patients and families.
To complete referrals to LCSW and substance abuse/use counseling services.
To update patient records at each visit including (but not limited to) address, telephone number, patient treatment agreement, medical release of information and to assist patient in identifying problems/needs, and potential resources to meet those needs.
To serve as a liaison between the patient, family, and CSC.
To serve as patient advocate when appropriate.
To attend all meetings and training and participate in a professional manner.
