Patient Care Coordinator

:
The responsibilities of the Patient Care Coordinator are to build fantastic rapport with the patients who\'s data and care they oversee, helping them feel loved and cared for. Additionally the PCC is to report all pertinent information to the correct medical professional as outlined by the medical director.
As technological problems may arise with deployed devices, the PCC is to attempt fixes, and then to follow the correct protocols to turn the tech issues over to the tech team for resolution.
Once a PCC has completed orientation and has a census of patients, this position becomes very flexible. Most team members work several days from home each week after the first few months, and this aligns with our culture of big accountability and big trust.
This position is 95% remote, but those applying should live in the Ogden area. The patients will be located in the Ogden area, and weekly visits are encouraged.
Essential Job Duties:

  • Maintain professional and friendly demeanor with each patient interaction
  • Have excellent knowledge of how to help the patient (will be trained on this).

  • Be able to build rapport with the patients during the regular interactions, and be able to remember personal information about each patient to use in relationship building (or be able to develop a system of writing information down so as to help each patient feel special and individually cared for.)
  • Deliver important information to other members of the team or to the appropriate medical professionals
  • Answer all patients\xe2\x80\x99 concerns and questions patiently and professionally.

Job Qualifications:
2-6 years Experience working with seniors or in a medical environment preferred
Age over 18 years
Excellent communication skills including the ability to speak, write and read English
Ability to operate computer, necessary software programs ( gmail, google docs, etc).
Ability to be professional at all times with patients, other team members, and others you interact with.
Universal Precautions:

Exposure to blood/bodily fluids is unlikely.
Physical Requirements:

Must be able to type on a computer and talk on a phone.
Must be able to travel to or arrange travel to each patient in your assigned patient pool as directed.
