Job Description

The Annual Wellness MA is responsible for implementing yearly annual wellness visits and preventive visits. The purpose of the wellness visit is to create a personalized prevention plan to reduce or prevent the risk of disease and to help the patient manage chronic conditions. This will include conversing with the patient to review medical and family history, reviewing medications, developing a health screening schedule, discussing advanced care planning, performing depression and cognitive screenings, and providing counseling to manage chronic conditions.

Prefer CMA with extensive office experience and direct patient care experience.
