Care Management Support Specialist (CMSS)

We are seeking a full-time Care Management Support Specialist (CMSS) to provide Chronic Care management (CCM), Remote Patient Monitoring (RPM), and Post-Discharge support (TCM) to patients with multiple comorbidities. This is primarily a telephonic position at our corporate location in Boynton Beach, FL.

Responsibilities:

Handles incoming calls from patients Reaches out to patients when needed Monitors and evaluates patient's vitals via RPM Communicates with Care Team and Primary Care Providers Schedules home visits for the Care Coordinators
Qualifications:

Certified/Registered Medical Assistant Prior experience working with CCM program highly desired Must possess excellent interpersonal, telephone communication, and customer service skills Bilingual English/Spanish preferred Experience with eClinical Works preferred
Individuals must pass a background screening and drug test

Job Type: Full-time

Pay: $17.00 - $22.00 per hour

Expected hours: 40 per week
