Job Description

LIV Health is seeking a dedicated and compassionate Medicaid Waiver Case Manager to join our team. This home-based position will manage a split caseload, with half based in Laramie and half in Cheyenne, providing services under both the Community Choices Waiver (CCW) and the Supports Waiver (DD waiver). We provide office equipment, and you will be reimbursed for mileage when using your own vehicle.

The Case Manager will support individuals in accessing, coordinating, and maintaining the services they need to thrive in their community. We are looking for a highly organized, compassionate, hardworking individual dedicated to making a positive difference in the lives of those we serve.

Responsibilities:



Provide case management services for participants enrolled in the Community Choice Waiver and Supports Waiver. Develop, implement, and monitor individualized care/service plans. Assist clients in accessing community resources, medical care, therapies, and support services. Ensure all documentation and plans comply with state and federal Medicaid regulations. Conduct home visits and community visits as required. Collaborate with families, providers, and interdisciplinary teams to ensure quality care. Advocate for client needs and support them in achieving their goals. Maintain accurate, timely records in accordance with waiver requirements.

Qualifications:



Bachelor's degree in Social Work, Human Services, Psychology, or a related field. Experience working with Medicaid Waiver programs, case management, or related human services is preferred. Strong organizational and time-management skills. Compassionate, empathetic, and dedicated to serving vulnerable populations. Ability to travel between Cheyenne and Laramie as part of a split caseload. Must pass background checks and meet state certification requirements for waiver case managers.

Schedule and Compensation:



Full-time equivalent (FTE), 40 hours per week. Caseload split between Cheyenne and Laramie. Competitive compensation, commensurate with experience.
