About Us

Peachtree Care Health Services (PCHS) is a Medicaid-approved home care agency dedicated to helping families access free home care through Georgia's Medicaid waiver programs. We're expanding our Referral Partner Network and seeking caseworkers, case managers, and social workers who are already in touch with Medicaid-eligible individuals.

As a Referral Partner, you'll leverage your existing connections to Medicaid clients and families. Simply connect eligible individuals to PCHS, and our intake team will handle the enrollment process.

This is a flexible, part-time opportunity that allows you to earn commission income while continuing the work you already do in your community.

This role is best suited for professionals who regularly serve or interact with Medicaid clients, including:

Case Managers & Care Coordinators Social Workers & Human Service Professionals Hospital & Rehab Facility Discharge Planners Community Leaders and Nonprofit Staff

Responsibilities

Refer Medicaid-eligible clients to PCHS home care services Share program information with families and clients Serve your local community ZIP codes while earning extra income

Compensation

$40-$60 per qualified referral (plus bonus opportunities) Flexible schedule - no required hours, fits alongside your current job An effortless way to earn supplemental income while helping families access the care they deserve
