We are currently seeking a dedicated and experienced MDS Coordinator to join our team at Elkader Care Center located in Elkader, IA. The MDS Coordinator is a vital member of our interdisciplinary team, responsible for assessing residents and developing care plans to ensure they receive the highest quality care.

Responsibilities:

Coordinate the development and completion of the MDS for each resident Collaborate with healthcare providers to develop comprehensive care plans Ensure accurate and timely documentation of resident assessments and care plans Maintain compliance with state and federal regulations regarding MDS assessments Participate in interdisciplinary meetings to discuss resident progress and adjust care plans as needed

Qualifications:

Active RN license in the state of Iowa Prefer 2 years experience as an MDS Coordinator in a long-term care setting Strong attention to detail and organizational skills Excellent communication and interpersonal skills Ability to work collaboratively in a team environment
