MDS Coordinator (RN/LPN) - Long-Term Care

Position Summary:

We are seeking an experienced and detail-oriented

MDS Coordinator (RN or LPN)

to join our Long-Term Care team. The MDS Coordinator is responsible for coordinating and completing accurate, timely MDS assessments in compliance with state and federal regulations while supporting quality resident care and reimbursement integrity.

Responsibilities Include:

Complete and submit MDS assessments (admissions, quarterly, annual, significant change, PPS as applicable) Ensure accuracy and timeliness of MDS submissions in accordance with CMS guidelines Coordinate Care Area Assessments (CAAs) and interdisciplinary care plans Monitor Medicare/Medicaid reimbursement and case mix accuracy Collaborate with nursing, therapy, social services, dietary, and physicians Participate in audits, surveys, and quality assurance activities Maintain compliance with facility policies and regulatory standards

Qualifications:

Active

RN or LPN license

in state of TN Prior

MDS experience in Long-Term Care preferred

Knowledge of RAI process, PDPM, and CMS regulations Strong organizational, analytical, and communication skills Ability to work independently and as part of an interdisciplinary team
