Administrative functions:


Assist in the collection of resident assessment data (MDS), in accordance with current rules, regulations, and guidelines, that govern the resident assessment, including the implementation of CAA and triggers. Review updates of written policies and procedures that govern the development, use and implementation of the resident assessment ( MDS) and care plan Participating in, an ongoing quality assurance program for the collection of resident data of resident care plans Participate in functions involving discharge plans as may be necessary Implements procedures with the RN resident assessment/care plan coordinator to inform all assessment team members of the arrival of newly admitted patients Develop the schedule of activities required for completion of the RAI process Assist with coordinating the development of a written plan of care ( preliminary and comprehensive) for each resident that identifies that problems/needs of the resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care. Conduct daily, concurrent utilization review via chart audit in order to ensure that change of condition RAI's are initiated when appropriate Conduct day concurrent utilization review via chart audits in order to ensure that discharge planning proceeds smoothly relative to resident's progress and Medicare coverage status. Main the

Qualifications

& Experience Requirements for

MDS Coordinator:



Previous Experience as a MDS Coordinator preferred Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) in the State of Connecticut required Must be an LPN or RN
