Job Summary:

Village Health Care LLC is currently looking for a

MDS Coordinator

to oversee the proper administration of total care to our residents. The ideal candidate will also assist with the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Area Assessments (CAAs) and Care Plan in accordance with State and Federal requirements.

Essential Functions:

Coordinate the IDT in timely completion of the assessments. Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs). Communicate ARD selection for specific MDSs to IDT. Complete Care Area Assessments (CAAs) as required. Complete care plan items relevant to current MDS and attend care plan meetings as assigned. Monitor the overall documentation in the medical record to validate that it supports MDS coding. Participate in Medicare utilization management as assigned. Stays up to date in RAI Regulation Other responsibilities as delegated

Education, Licensure, and Experience:

Required:Graduate of an accredited school of nursing. Required:Current LPN or RN licensure in the state of employment. Required: Computer aptitude with the EMR for this position Preferred: Two years' experience working in a long-term care Preferred: Experience with Residence Assessment Instrument (RAI) process and MDS certification Preferred: Point Click Care (PCC) experience Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
