We are seeking a dedicated and detail-oriented MDS Coordinator to join our skilled nursing team!

As an MDS Coordinator at our facility, you will play a vital role in ensuring the accuracy of resident assessments, optimizing quality of care, and supporting the interdisciplinary team through precise documentation and coordination. Your expertise helps ensure our residents receive the best possible outcomes -- and our facility maintains the highest standards of care and compliance.

MDS Coordinator Duties, Responsibilities & Qualifications:

Complete and submit Minimum Data Set (MDS) assessments in accordance with federal and state regulations and facility policies. Coordinate and lead interdisciplinary care plan meetings to ensure all resident needs are accurately addressed. Monitor, review, and update care plans based on ongoing assessments and resident status changes. Ensure accuracy and timeliness of all documentation to support reimbursement and compliance standards. Collaborate with nursing, therapy, and social work teams to maintain continuity of care. Maintain current knowledge of RAI guidelines, Medicare/Medicaid regulations, and quality measures. Hold an active, unencumbered Registered Nurse (RN) or Licensed Practical Nurse (LPN) state license. Demonstrate strong attention to detail, critical thinking, and organizational skills. Prior experience as an MDS Coordinator or with MDS 3.0 in a long-term care setting preferred.
