POSITION SUMMARY

: The Program Coordinator supports the Community Health and Outreach departments in engaging the community in services and activities that provide education, outreach and support as required to accomplish the scope of work for the Community Health and Outreach Programs. The Program Coordinator is a health advocate for the Native American community in coordination with other departments of Northern Valley Indian Health and community agencies. The Program Coordinator performs a variety of health-related activities that involve assessing patient needs, developing appropriate plans of care, assisting patients in accessing services, providing basic health promotion and disease prevention education, maintaining a patient caseload, and accurately documenting care coordination services. Program Coordinator duties and scope may be focused or limited based on grant funding.


ESSENTIAL JOB FUNCTIONS

The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) will be required to perform a variety of duties listed, including but not limited to:


Develop and implement patient centered programs that establish community partnerships to support the mission of the coalition and collaborative services based on the needs of the communities we serve. Ensure program compliance with NVIH policies and procedures as well as applicable local, state, and federal regulations. Evaluate and analyze effectiveness of programs and partner with department directors and others to continuously improve programs. Communicate and report on the status and effectiveness of programs with various stakeholders, as directed. Conduct and/or facilitate culturally appropriate group/individual education sessions to NVIH staff, patients, families and communities on program initiatives. Support programs and other assignments involving program funding, including but not limited to budgets, grants, etc. Other duties as assigned.


Experience and Skills:

REQUIRED QUALIFICATIONS



The following knowledge, skills, and abilities are required for an employee to successfully perform their duties.



+ High school diploma or equivalency / GED.
+ Two (2) years' experience working with underserved population in a Community Outreach capacity. Will consider volunteer work or formal education/training for work experience.
+ Knowledge of program scope based on agency need
+ Effective written and verbal communication skills and ability to speak publicly, such as conducting group trainings.
+ Proven ability to engage in culturally sensitive geographic areas.
+ Able to effectively work independently with minimal supervision.
+ Able to travel for training and maintain a valid California driver's license and insurability with the agency auto-policies along with maintaining reliable transportation.
+ Able to think critically and apply problem solving techniques; willing to learn and apply health training program(s).
+ Basic computer skills required; internet browsing/researching, typing, email, etc.
