Upper Great Lakes Family Health is seeking a

MAT Care Manager

to provide care coordination and support to patients enrolled in the Medication-Assisted Treatment (MAT) program for substance use disorders. This role focuses on improving patient engagement, treatment adherence, and recovery outcomes through patient-centered care, collaboration with multidisciplinary teams, and consistent monitoring and follow-up.

PROFESSIONAL/TECHNICAL KNOWLEDGE, SKILLS & ABILITIES

Minimum of two years' experience in addiction medicine, behavioral health, or care management.

Demonstrates an understanding of FQHCs.

Demonstrates an understanding of PCMH model of care.

Knowledge of substance use disorders, recovery models, and MAT protocols (e.g., buprenorphine, naltrexone, Sublocade, Vivitrol, Brixadi).

Proficient in motivational interviewing and trauma-informed care.

LICENSES & CERTIFICATIONS

Current unrestricted Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN), or equivalent clinical credential.

CPR-BLS Certification

TECHNICAL SKILLS

Microsoft Office Suite to create correspondence, tables, charts, graphs, spreadsheets, and presentations.

Adobe Acrobat to create and manipulate forms. Electronic clinical systems, including EHR and scheduling systems in use at the health center.
