Bachelor's Degree in Nursing required; Master's Degree preferred.
Current Massachusetts RN License.
Minimum 5 years of acute-care Case Management experience.
At least 2 years of experience in process improvement.
Experience developing and presenting continuing education programs.
Knowledge of CMS regulations.
Strong computer skills, including Windows-based systems.
Excellent organizational, analytical, writing, and interpersonal skills.
Preferred:
Certification in Case Management or Clinical Documentation Integrity.
Prior management or supervisory experience.
Key Responsibilities:
Operational & Staff Management
Oversee daily operations of Case Management and Social Work teams, including staffing, scheduling, and performance evaluations.
Support Clinical Documentation team functions as needed.
Conduct rounding on patient units to identify workflow barriers and expedite discharge processes.
Education, Training & Professional Development
Design and deliver continuing education programs for Clinical Resource staff.
Provide onboarding for new hires, including Case Managers, Social Workers, physicians, nurses, and allied health staff on discharge planning and utilization review best practices.
Program Monitoring, Compliance & Improvement
Track, analyze, and report program compliance and develop dashboards to identify improvement opportunities.
Implement process improvement initiatives based on performance metrics.
Maintain knowledge of regulatory changes related to case management and utilization review.
Mentor and coach clinical teams to enhance collaboration between physicians, nursing staff, and Clinical Resource members.
Promote a culture of continuous improvement by recommending workflow enhancements.
Actively support organizational goals and best practices in patient care.
