Scheduled hours - 36 hrs
Contract Duration - 13 weeks

Job Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our long-term care facility. The LTC LPN will provide comprehensive patient care, assist with medical assessments, and support the health and well-being of residents across various care settings. The ideal candidate will have extensive experience in geriatrics, memory care, and acute or chronic illness management, ensuring high-quality nursing services in a compassionate environment.

Duties

Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations. Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and safety protocols. Manage IV infusions, catheter care, and ventilator support for patients requiring advanced nursing interventions. Document all patient interactions and treatments thoroughly using EMR/EHR systems like Epic or Cerner. Assist with diagnostic evaluations such as laboratory specimen collection, processing, and imaging procedures. Provide specialized care for patients with dementia, Alzheimer's disease, behavioral health needs, and those requiring memory or hospice care. Support discharge planning, case management, and coordination of post-acute or home & community care services. Educate patients and families on health coaching topics such as nutrition, medication management, and chronic disease management.

Experience

Minimum 2 years of experience required. Active State of HI license. BLS
