We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care (LTC) team. The ideal candidate will have experience in various healthcare settings, particularly in long-term care, assisted living, and geriatrics. This role requires a strong commitment to providing high-quality patient care and supporting residents with diverse needs, including those with developmental disabilities and memory care challenges.
Responsibilities
Provide direct patient care to residents in accordance with established care plans and protocols.
Administer medications and treatments as prescribed, ensuring adherence to safety standards.
Conduct comprehensive patient assessments, including vital signs monitoring and intake evaluations.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Maintain accurate medical documentation in compliance with HIPAA regulations.
Assist with daily living activities, including tube feeding, catheter care, and personal hygiene.
Support residents with Alzheimer's and dementia care, focusing on enhancing their quality of life.
Engage in patient education regarding health management and wellness strategies.
Participate in discharge planning and case management for residents transitioning out of the facility.
Ensure a safe environment by adhering to infection control protocols and performing routine safety checks.
Qualifications
Current LPN license in good standing.
Experience in long-term care, assisted living, or geriatrics is preferred.
Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is advantageous.
Critical care experience or exposure to acute pain management is a plus.
Knowledge of ICD-9/ICD-10 coding and medical terminology is beneficial.
Strong skills in patient assessment, medication administration, and basic life support techniques.
Ability to work effectively with individuals with developmental disabilities or complex medical needs.
Excellent communication skills and a compassionate approach to patient care.
