We are seeking an experienced LPN or Registered Nurse to become our next Nurse Care Manager

Schedule & Benefits
This is a full-time role, with the expectation of 40 working hours per week, though days and times may vary according to business needs. Participation in our on-call rotation is required, with a minimum commitment of one weekend per month or equivalent. Work will take place in a combination in our office, out in the field within our service area, and your home or other remote location.

Job Responsibilities
Primary responsibilities include but are not limited to:
1. New Clients
Scheduling and performing prospective new client assessments
Consultative selling to convert leads to clients
Writing and maintaining Service Agreements and Care Plans in compliance with state regulations
2. Caregiver Preparedness
Participating in New Caregiver Orientation, including skills evaluation and training
Briefing caregivers and facilitating client/caregiver introductions
Supervising, supporting and coaching caregivers
3. Quality Assurance
Monitoring shift notes and client conditions
Performing supervisory visits and unannounced quality assurance visits to assess quality of care and client satisfaction
Tracking and reporting incidents, compliments and complaints
4. Community Relations
Establishing and nurturing relationships with referral sources
Participating in networking events
Contributing to marketing efforts
Additional responsibilities include providing care coordination for clients enrolled in this
standalone service, ensuring regulatory compliance, performing miscellaneous administrative duties and other tasks as assigned.

Minimum Requirements
Valid Vermont LPN or Registered Nurse (RN) license
Valid driver's license and reliable transportation
Excellent communication and interpersonal skills
Strong initiative, accountability and service-orientation
Proficiency with Microsoft (Word, Excel, PowerPoint) and Google (Gmail, Calendar,
Drive) programs
At least 5 years of nursing experience
Preferred Qualifications / Experience
Experience in non-medical home care, home healthcare and / or home hospice strongly preferred
Clinical and administrative nursing experience
Supervisory experience
