New Haven Center for Nursing and Rehabilitation is seeking LPNs to join our team!



We are currently hiring for both full and part time positions.



Must have an active CT LPN License and be CPR Certified.



Shift and weekend differential!



This is a 1199 union position. The right candidate must be a great communicator and communicate with staff and management appropirately.

I.

ESSENTIAL JOB FUNCTIONS:

A.

Direct the day-to-day functions of the nursing activities and accordance with current rules, regulations, and guidelines that govern the long-term care facility.

C.

Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.

E.

Admit, transfer, and discharge residents as required. Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, if applicable, needed medical supplies, etc. Ensure that the room is ready for new admissions. Greet newly admitted residents upon admission. Escort to room as necessary.

I.

Accurately transcribe physicians' orders to resident charts, kardex, treatment/care plans as required.

J.

Report all discrepancies noted concerning physician's orders, diet change, charting errors, etc., to shift supervisor.

K.

Perform routine charting duties as required and in accordance with our established charting and documentation policies and procedures.

L.

Prepare and administer medications as ordered by the physician.

M.

Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies.

N.

Ensure that narcotic records are accurate for your shift following current policy. Dispose of drugs and narcotics as required with established procedures.

O.

Review medication administration records for completeness of information, accuracy in the transcription of the physician's orders, in adherence to stop order policy.

P.

Notify the shift supervisor of all drug and narcotic discrepancies noted on your shift upon discovery.

Q.

Develop work assignment, and/or assist staff in completing and performing such tasks. Make several rounds (at least every two hours) on your unit to ensure that nursing services personnel are performing their work assignments in accordance with acceptable nursing standards. Report problem areas to supervisor.

S.

Make independent decisions concerning nursing care, after assessing the resident's physical status. Inform shift supervisor and MD and family as necessary.

A.

Coordinate efforts with the staff nurse to assure the prescribed treatments are being properly administered and to evaluate the residents' response and physical and emotional status daily and as ordered including documentation weekly on all skin impairments.

B.

Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, give massages and range of motion exercises, give care to the terminally ill residents. Provide direct nursing care etc., as necessary or required.

C.

Report problem areas to the shift supervisor. Assist in developing and implementing corrective action. Ensure that the plan of care is carried out shift to shift.

D.

Keep the shift supervisor informed of the status of residents and other related matters through written or oral reports.

E.

Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.

F.

Monitor residents listed on report that are experiencing a change in condition.
