Mosaic is looking for a Licensed Practical Nurse (LPN) to join our team!

In this role, you will provide health care to individuals receiving services in the homes by scheduling medical appointments, assessing condition, reviewing medications, completing any necessary medical forms and by updating medical records.

What you will do:

• Coordinate physician and health care services for people served
• Conduct health care assessments and write health care plans
• Facility training and orientation of new staff regarding healthcare procedures
• Provide on call assistance to staff as needed

SCHEDULE: PRN / ONCALL \xe2\x80\x93 24 hours a month required \xe2\x80\x93 1 Day shift (7a-7p) and 2 Night (7p-7a) shifts \xe2\x80\x93 no benefits - NEW GRADUATES WELCOME!

Requirements

What YOU\xe2\x80\x99LL Need:

• State license required in nursing
• 1 year of general nursing experience \xe2\x80\x93 NEW GRADUATES WELCOME!
• Must be able to lift, push and pull at least 50 pounds.
• Valid Driver\'s license
