Schedule: Mon-Fri 8:30am-5:00pm

Territory: Massachusetts Office Location Newburyport, MA

Position Summary: The LPN Intake functions under the direction of the Intake Supervisor for the timely and accurate intake of the initial home care or hospice plan of treatment /care referral to the agency from referral sources in compliance with regulatory, quality, industry, and Constellation standards.

Position Requirements:

  • Must be a graduate of an accredited LPN program.
  • Current LPN license and maintenance of license in the State of Massachusetts.
  • Minimum of 2 years clinical nursing experience, home care experience preferred.
  • Excellent communication, interpersonal, computer and customer service skills.
Responsibilities:
  • Under the direction of the RN Intake Coordinator, the LPN Intake screens, triages, and processes requests for agency services inclusive of admission criteria, discipline, and programs, utilizing and demonstrating a solid understanding of home care criteria, and payment sources.
  • Assesses the appropriateness of each referral according to Agency policies and procedures and in collaboration with the primary physician and referral sources.
  • Utilizes all computerized referral source programs and reporting programs timely and effectively.
  • Facilitates and ensures quality, coordinated and timely clinical services as the patient moves across the continuum of care.
  • Oversees and assists with the coordination of workflow processes as assigned.
  • Identifies and communicates priority visit needs and potential high-risk situations to the designated supervisor to facilitate timely and appropriate scheduling of professional and support staff and services.
  • Ensures all pertinent data is accurate and complete and utilizes all available information and resources to promote quality care and the achievement of patient goals and outcomes.
  • Demonstrates knowledge of home care processes in all communications with referral sources.
  • Works collaboratively with scheduling, billing, and quality departments to ensure and facilitate the home care services comply will all regulatory and agency guidelines.
  • Communicates effectively with all internal and external contacts utilizing excellent customer service skills.
  • May attend appropriate agency and community meetings as assigned and directed.
  • Will perform other duties as assigned.
