Midland Care is currently seeking a dedicated and skilled Licensed Practical Nurse (LPN

)

to provide high-quality hospice care in a home setting. This position plays a key role in supplementing the nursing care provided by the RN and ensures the delivery of patient-centered, end-of-life care with dignity, empathy, and professionalism.

Employment Type:

Full Time | M-F 8am-5pm

Pay Rate:

$26/hr+ (pay is based on qualifications and experience)

Topeka, KS

Key Responsibilities:

Deliver direct patient care under the guidance of a Registered Nurse and in alignment with the patient's plan of care. Monitor, report, and document changes in patient condition; collaborate with the interdisciplinary team. Educate patients, families, and caregivers on care techniques and safety measures. Assist with personal care activities including hygiene, mobility, and basic home maintenance.

Qualifications:

Current LPN license in the state of Kansas. Minimum of one year experience with chronically ill and/or terminally ill patients. Valid Kansas Driver's License and proof of auto liability insurance. Reliable transportation and an acceptable Motor Vehicle Record per company standards.
