Job Overview


We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) Floor Nurse to join our healthcare team on a full-time basis, working the 2:00 PM to 10:00 PM shift. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings, including long-term care, assisted living, and acute care environments. This role offers an opportunity to provide high-quality nursing services, support patient recovery, and enhance the well-being of our diverse resident population. The LPN will be responsible for delivering comprehensive nursing care, ensuring adherence to safety protocols, and collaborating effectively with interdisciplinary teams to promote optimal health outcomes.

Responsibilities



Perform thorough patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses to treatment plans. Administer medications and injections accurately while adhering to established protocols and maintaining detailed medical documentation in electronic health record (EHR) systems such as Epic or Cerner. Manage IV infusions and catheter care with precision, ensuring sterile technique and patient safety at all times. Assist with complex procedures including wound suturing, tube feeding, and airway management in accordance with aseptic techniques. Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care and behavioral health interventions. Support acute pain management strategies through careful assessment and appropriate medication administration. Collaborate with physicians and interdisciplinary teams during diagnostic evaluations such as medical imaging or laboratory specimen collection. Participate in discharge planning, case management, and patient education to promote continuity of care within outpatient or inpatient settings. Maintain compliance with HIPAA regulations by safeguarding patient confidentiality during all documentation and communication activities. Demonstrate proficiency in ICD-9/ICD-10 coding for accurate medical records management and billing processes. Engage in infection control practices to prevent healthcare-associated infections across various clinical environments including hospital medicine and post-acute care settings. Utilize skills in triage to prioritize patient needs efficiently within emergency room or urgent care contexts. Provide compassionate support for patients with complex needs such as hospice care or those requiring ventilator management.

Requirements



Valid LPN license from an accredited nursing program with current state licensure. Prior experience in diverse clinical settings such as long-term care facilities, hospitals (including ICU or Level I Trauma Center), assisted living communities, or home health environments. Demonstrated expertise in medical-surgical nursing, geriatrics, pediatrics, or behavioral health is highly desirable. Proficiency in electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; experience with medical documentation is essential. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment planning. Ability to perform venipuncture, phlebotomy, sterile processing procedures, and specimen collection/processing accurately. Experience administering injections and managing IV lines with aseptic technique; familiarity with IV insertion is required. Excellent communication skills for effective collaboration with multidisciplinary teams and working with diverse populations including individuals with disabilities or developmental delays. Knowledge of infection control standards and OSHA regulations to ensure a safe environment for patients and staff alike. Physical ability to assist patients using lifts such as Hoyer lifts; capable of performing physical examinations requiring mobility assistance when necessary.
