Job Title: LPN Health Care Coordinator



Department: Nursing



Reports to: Program RN & ADNS





Working under the direction of the program Registered Nurse, is responsible to assist in the management of the health, safety and well being of intellectually disabled residents in residential facilities and day programs.

Essential Job Functions





Conduct self in a professional manner when interacting with participants, supervisors, co-workers, families, medical professionals and members of the community


Create a nurturing, positive learning environment and develop strategies to help the clients cope effectively with social, emotional and physical situations of daily living and facilitate independent performance in these areas.


Assists the RN by providing data, contributing to the identification of priorities and contributing to the identification of realistic and measurable goals.


Assures individuals' right to privacy and adheres to CRI's confidentiality policy.


Familiarize oneself with CRI policies and procedures, ensuring compliance with same.


Keeps RN advised in all aspects of the implementation of the health plan, the person's response to the plan and changes in prescribed treatments that may impact the plan.


Supports the RN evaluation through communication of observations, data and person's response to treatment.


Supports the assessment process through collection of data, reporting of pertinent observations to the RN and suggestions of nursing intervention modification based on the person's response.


Reports directly to the RN any change in condition of the person, a new or changed medication prior to implementation, a new order or change in treatment prior to implementation and any and all medical concerns or issues.


Monitors medical, physical and behavioral changes of residents. Reports all changes to the RN and Program Coordinator/Director as appropriate.


Documentation of all issues, communications, changes in conditions in nurses' notes on a daily basis.


Keeps current with new medical information by attending in-services, conferences and accessing medical information through reading materials and the internet.


Participate in training opportunities (in-service training, staff meetings, conferences, etc.), as required; maintain all required training per agency policy and DDS regulatory requirement.


Authorized LPN may be responsible for annual and recertification on site practicum for non licensed medication certified staff per DDS#NS 09.3.


May delegate, under the direction of the RN, all or portions of the implementation of the health care plan to appropriately trained non-licensed personnel per DDS standards.


May be responsible for retraining medication certified staff following medication errors, if concept previously taught by an RN.


May be responsible, under the direction of an RN, for Quarterly Review of Medication Administration Reports.

Position Requirements





Must have a high school diploma, or a GED; must be a graduate of an accredited LPN program with current CT LPN license Must possess a valid CT driver's license, be able to drive own and agency vehicles and have a good driving record. Must be an LPN with three years experience in a medical setting. Must have two years experience in the DD field. Must maintain a current LPN license and submit to agency current license annually Must be eligible to become authorized by DDS for the medication administration certification process Must maintain authorization in medication administration certification process. Knowledgeable of LPN Scope of Practice Must maintain medical training and in-servicing includes, but is not limited to, OSHA and CPR recertification. Possess good to excellent oral and written communication skills. Demonstrate thoroughness and follow through on daily work assignments. Display motivation and interest in improving quality of life for population served. Demonstrate and maintain an optimistic attitude, patience and compassion for and with the population served. Demonstrate the ability to organize and prioritize medical responsibilities with direct care responsibilities to completion.
