Key Responsibilities
Administrative Duties:
Assist with patient intake, start-of-care coordination, and scheduling
Review and maintain accurate patient records and clinical documentation
Ensure compliance with state, federal, and agency regulations
Communicate with physicians, RN's, HHA's, patients, and family members
Support care coordination, authorizations, and follow-up documentation
Assist with audits, re-certifications, and quality assurance activities
Skilled Nursing Duties:
Perform skilled nursing visits under RN supervision (as permitted by state regulations)
Monitor vital signs and patient condition; report changes to RN/physician
Administer medications and treatments as ordered
Provide wound care, catheter care, and other skilled services
Educate patients and caregivers on care plans and disease management
Ensure patient safety and infection control in the home setting

Qualifications:
Current
LPN license
in good standing in the state of employment
Minimum
1-2 years of nursing experience
(home health preferred)
Strong knowledge of home health regulations and documentation standards
Excellent organizational, communication, and time-management skills
Proficiency with EMR systems and basic computer skills
Valid driver's license and reliable transportation
Ability to work independently and as part of a team
