Job Summary

The Long Term Care Coordinator is responsible for the oversight and coordination of care for all long term care residents. This includes leading care conferences for all Long term care residents, Restorative Nursing responsibilities, assisting with CNA assignments, Pharmacy recommendations. The Long Term Care Coordinator will model professional, high quality care, communication with between professionals, as well as promote a team environment on the unit. This person will also participate in the "on call" rotation and must be able to work as a charge nurse on any unit when and if needed.

Qualifications

The Long term Care Coordinator must be a Licensed or Registered Nurse currently licensed in the State of Rhode Island.

LTC experience is necessary.

This Nurse must display a strong sense of responsibility and be able to function effectively and efficiently without or with little supervision.

The Long Term Care Coordinator must demonstrate maturity and have the ability to deal with personnel, patients and family effectively in a professional manner.

Pay: From $40.00 per hour

Expected hours: 40.0 per week
