You will be a vital part of enhancing quality of life, promoting independence, and ensuring safety.
Duties
Assist client with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Monitor patient health by checking vital signs such as blood pressure, pulse, and respiration; observe and report any changes or concerns
Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance
Support client with meal preparation, nutrition, and hydration to promote overall well-being
Use specialized equipment such as Hoyer lifts to safely transfer client with mobility challenges or disabilities
Maintain detailed documentation of care provided, patient observations, and any incidents in accordance with privacy standards like HIPAA
Provide companionship and emotional support to reduce feelings of loneliness or isolation
Assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe environment
Collaborate with healthcare professionals to implement individualized care plans tailored to each client's needs
Support clients in community outings, Doctor's appointment, Physical Therapy/Occupational therapy to foster engagement and independence
Follow all safety protocols including CPR and First Aid procedures to ensure client safety at all times
Experience
Prior experience working in home health care, assisted living facilities, nursing homes, or group homes is highly valued
Demonstrated ability to work effectively with individuals with developmental disabilities, dementia, Alzheimer's disease or memory impairments
Knowledge of patient observation techniques and monitoring vital signs accurately
Experience assisting with medication administration and understanding care plans is preferred
Familiarity with HIPAA regulations ensuring confidentiality and privacy in all interactions
Proven ability to assist with transfer or mobility assistance
Strong communication skills for collaborating with families and healthcare teams effectively
