Qualifications

A Licensed healthcare professional with experience in providing functionally based assessments and skills training for individuals who has transitioned to the community from an institutional setting, such as a Skilled Nursing Facility Knowledge of the development of plans for client care and the ability to participate in them Ability to recognize the needs and concerns of people, and to maintain constructive working relationships in dealing with them Ability to communicate effectively both orally and in writing Valid, current State driver's license Bachelor's (Required)

Summary

As a Life Skills Trainer, you will be instrumental in empowering individuals to achieve greater independence and improve their quality of life. Reporting to the program supervisor, you will utilize your core skills in home and community care, direct support, and communication to assist clients with daily living activities, such as budgeting, goal setting regarding household activites, managing doctor appointments, Social Security benefit assistance, etc. Additionally, your background in social work and supervisory experience will be vital in fostering a nurturing environment that promotes personal growth and development.

Responsibilities

Provide direct support to individuals who have recently transitioned to the community from a Skilled Nursing Home. You will be assisting them in daily tasks to promote independence. Following the members care plan, you will focus on tasks that will enhance their daily life skills to promote independence. Foster effective communication and build rapport with clients and their families. Document progress and maintain accurate records of client interactions and outcomes.

Pay: $23.00 per hour
