We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care facilities. As an LPN, you will play a vital role in supporting patients' health and well-being while collaborating with healthcare professionals to ensure optimal outcomes.
Responsibilities
Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities.
Conduct patient assessments and monitor vital signs to evaluate health status.
Collaborate with registered nurses and physicians to develop and implement individualized care plans.
Maintain accurate medical documentation in compliance with HIPAA regulations and facility policies.
Assist in triaging patients and managing acute pain effectively.
Support patients with developmental disabilities, Alzheimer's care, memory care, and dementia care.
Perform IV infusion therapy, catheterization, phlebotomy, and other nursing procedures as needed.
Educate patients and families on health management strategies and provide health coaching when applicable.
Ensure infection control protocols are followed during all patient interactions.
Experience
Candidates should have experience or knowledge in the following areas:
Critical care experience in Level I or Level II trauma centers
Medical office experience with familiarity in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks
Long-term care experience in nursing homes or assisted living facilities
Hospital experience including emergency medicine, ICU, PICU, or medical-surgical units
Familiarity with ICD-9 and ICD-10 coding for accurate medical documentation
Experience working with seniors in hospice or home care settings
Knowledge of anatomy and physiology related to patient assessment and management
Proficiency in performing physical examinations, diagnostic evaluations, medication administration, and basic life support
