Job Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including long-term care, assisted living, and acute care environments. The LPN will work closely with registered nurses and other healthcare professionals to ensure the well-being of patients and support their medical needs.

Duties

Administer medications and treatments as prescribed by physicians. Perform patient assessments and monitor vital signs to evaluate patient conditions. Assist in the development and implementation of individualized care plans. Provide personal care services, including bathing, grooming, and feeding assistance. Conduct triage assessments in emergency situations and provide immediate care as needed. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to coordinate patient care effectively. Educate patients and families on health management, medication administration, and treatment plans. Utilize electronic health record (EHR) systems for documentation and patient management. Participate in infection control measures and maintain a clean, safe environment for patients.

Experience

Candidates should have experience or familiarity with the following areas:

Epic or Cerner EHR systems for efficient patient management. Critical care settings such as ICU or Level I/II trauma centers. Long-term care facilities, nursing homes, or assisted living environments. Medical office experience with knowledge of ICD-9/ICD-10 coding practices. Hospice care, geriatrics, or working with individuals with developmental disabilities. Experience in acute pain management, emergency medicine, or pediatrics is a plus. Proficiency in basic life support (BLS), phlebotomy, venipuncture, catheterization, and medication administration techniques.
